Keeping a complete record of a patients clinical data and medical history, we accurately transcribe and organize medical histories, symptoms and diagnoses, as well as categorize treatments and procedures.

If you would like a copy of your medical records, you must complete a medical records release form and forward it to us in person, by mail, or via fax. The form must be completed, filled out, signed, and dated by the patient or legal representative. Please allow 10 business days processing time after the form has been received by Medical Records.
